Health Requirements for Summer School
State of Illinois law requires school districts to have certain health information on file for students entering Summer School including verification of immunizations as well as physical and dental examinations. Students who are not in compliance with the health form requirements will not be allowed to participate in Summer School at ETHS until requirements are met.
The school-based ETHS Health Center (H101) is available for exams and immunizations to eighth-grade students who will be attending ETHS. Contact the Health Center at (847) 424-7265 to schedule an appointment.Visit the Health Requirements and Forms web page to view or download school health forms. Please return the completed and signed "Illinois Certificate of Child Health Examination Form" to the Nurses' Office (N121) by the "Health Forms Due" deadline listed on the Summer School Deadlines and Events calendar.
