Self-Certification Process for Students

  • In accordance with Illinois State Board of Education (ISBE) and Illinois Department of Public Health (IDPH) 2020-21 School Year guidelines, Evanston Township High School requires temperature and symptom screening of all staff, students, and visitors before entering the building or arriving on the ETHS campus to participate in extracurricular activities.

Students can now complete the COVID-19 symptom screening questions online. Self-certification will speed up entry to and participation in activities.

The self-certification application requires you to check your temperature. Confirmation of your completed temperature check is one of the COVID self-certification questions.

When you arrive at ETHS, please follow directions from your sponsor for admittance. Staff will scan student IDs to verify completion of the COVID self-certification.

Students who do not complete the self-certification must complete the on-site health symptom screening and temperature check.

Students can access the COVID self-certification screen by logging into myETHS. Click the COVID icon at the upper right, next to their username. (If using a small mobile device, click on the three-line “hamburger” menu icon to access the COVID icon.) Instructions for completing the Self-Certification, with screen images, are available to students online.

Self-certification can be completed only one time per calendar day and is valid until midnight of the date completed.
